This form should be submitted if you have lost an academic or activity leadership scholarship and would like to appeal due to unforeseen circumstances which caused you to drop below minimum scholarship requirements.

*Student athletes with a cumulative GPA below 2.8 are not allowed an appeal.

Scholarship appeal deadlines:

• Fall 2025 – deadline August 18, 2025
• Spring 2026 – deadline January 7, 2026
